Safe, Monitored Treatment Plans
Testosterone replacement therapy (TRT) is a physician-supervised treatment that restores testosterone to a healthy range in men whose levels have dropped below normal — whether from aging, chronic stress, poor sleep, weight gain, or an underlying medical cause. At Alturas Medical Weight Loss, TRT begins with a comprehensive lab panel, never a questionnaire, and every plan is prescribed and monitored by a licensed provider. If you have been looking for a Boise TRT doctor who reviews your bloodwork before writing a prescription, that is how we practice.
The goal isn’t to turn back the clock. It’s to help you feel like yourself again: clearer-headed, more energetic, and more in control of your body composition. Most men who come to us for TRT in Boise and Meridian aren’t chasing peak performance — they want their baseline back.
TRT at Alturas — the short version
What it treats: Clinically low testosterone (low T), confirmed by bloodwork.
Who it’s for: Men in Boise, Meridian and across the Treasure Valley with fatigue, low libido, brain fog, stalled gym progress, or unexplained weight gain.
How it’s delivered: Hormone pellets are our primary method; injections and topical options are available where appropriate.
What’s required first: A consultation and a comprehensive lab panel. We do not prescribe testosterone without confirming your numbers.
Who prescribes it: Dr. Ashley Southern, NMD, board-certified Naturopathic Medical Doctor and co-founder of Alturas.
Where: 1532 W Cayuse Creek Dr, Meridian, ID 83646 — about 15–20 minutes from downtown Boise. Call (208) 690-2040.
Pellet therapy is our primary method of delivering testosterone. A small pellet, roughly the size of a grain of rice, is placed under the skin of the upper hip during a quick in-office procedure. It releases testosterone steadily over several months, which avoids the peaks and crashes some men experience with weekly injections and the daily routine that gels require.
Pellets are not the right fit for every patient. During your consultation we walk through the options, what your labs actually show, and what realistically fits your schedule. Some men prefer injections, and that is a reasonable path. What we will not do is hand you a protocol before we have seen your numbers.

Our TRT patients are active men — weekend golfers, Foothills hikers, dads who want the energy to keep up with their kids — not bodybuilders chasing maximum muscle. If you have noticed persistent fatigue, stalled progress at the gym despite consistent effort, low motivation, poor sleep, added belly fat, or a dip in libido, TRT may be worth exploring.
It is also worth knowing what TRT is not for. If your labs come back in a healthy range, we will tell you, and we will look at the other things that could be driving your symptoms rather than writing a prescription you don’t need. Not sure whether what you’re feeling is low T? Our low testosterone symptoms page breaks down the most common signs.
Daily gels and weekly injections both work, but each comes with trade-offs. Here is how the three most common delivery methods compare:
| Method | How often | Level consistency | Best suited to |
|---|---|---|---|
| Pellets | Every few months | Most consistent | Men who want steady levels without a daily or weekly task |
| Injections | Weekly or twice weekly | Peaks and troughs between doses | Men comfortable self-injecting who want dose flexibility |
| Topical gels | Daily | Varies with absorption | Men who prefer no needles and don’t mind a daily routine |
Gels also carry a transfer risk — testosterone can rub off onto a partner or child through skin contact — which is one reason many of our patients rule them out. Pellet therapy is designed to provide more consistent hormone levels over time without a daily routine, and it is what most Alturas TRT patients end up choosing.
To understand what TRT can realistically do for energy, mood, sleep and body composition, see our page on the benefits of testosterone replacement therapy. If you are still trying to work out whether what you’re feeling is low T, our low testosterone symptoms and hormone imbalance symptoms in men pages can help you put words to it.

Dr. Ashley Southern, NMD
Physician and co-founder, Alturas Medical Weight Loss — Meridian, Idaho. Board-certified Naturopathic Medical Doctor supervising every TRT plan at the clinic.
Dr. Ashley Southern, NMD is the physician and co-founder of Alturas Medical Weight Loss in Meridian. She is a board-certified Naturopathic Medical Doctor with more than five years of clinical experience and annual certification in BHRT pellet therapy — the same delivery method used for most of our TRT patients.
She built Alturas because she was tired of watching men get handed a testosterone prescription with no real lab work, no follow-up, and no one who knew their name. Every TRT plan here is developed under her supervision, built around your bloodwork and your goals, and adjusted as your levels respond. Your medical weight loss and hormone care stay under one roof, with one clinical team.

The only reliable way is a blood test. Symptoms like fatigue, low libido, brain fog and mood changes can point to it, but we confirm with a comprehensive lab panel before recommending treatment. You can review the most common signs on our low testosterone symptoms page.
Yes. We do not prescribe testosterone based on a symptom questionnaire alone. Your first visit includes a consultation and a comprehensive lab panel so we can confirm clinically low testosterone, rule out other causes, and establish a baseline to measure against. Follow-up labs are part of the plan, not an optional add-on.
Dr. Ashley Southern, NMD, board-certified Naturopathic Medical Doctor and co-founder of Alturas Medical Weight Loss, oversees every treatment plan. She holds annual BHRT certification in pellet therapy and has treated more than 4,000 patients across multiple practices. You can read her full background on the Meet Dr. Ashley Southern page.
TRT is one part of men’s hormone replacement therapy. TRT specifically addresses low testosterone; broader men’s hormone therapy can also involve thyroid, cortisol and other hormones that affect energy, sleep and body composition. Your labs determine which conversation you actually need to have.
The insertion procedure is quick and performed under local anesthetic. Most patients describe mild soreness at the site for a day or two afterward and return to normal activity the same day, with a short pause on heavy lower-body exercise.
Pellets typically release testosterone steadily for several months before a follow-up insertion is needed. Your clinical team confirms timing based on your labs and how you respond, rather than on a fixed calendar.
Low testosterone is associated with increased body fat and reduced muscle mass, so restoring healthy levels can make it easier to build and hold lean mass. TRT is not a weight loss medication on its own. For men whose primary goal is weight loss, we often pair hormone care with our medical weight loss program. Individual results vary.
No. TRT at physiologic, medically supervised doses is designed to restore healthy testosterone levels, not produce dramatic muscle gain. Most patients notice improved energy and body composition over time, not extreme changes.
As with any hormone therapy, TRT carries potential risks and is not appropriate for everyone. That is why treatment starts with labs and continues with monitoring. We review your history, discuss the trade-offs openly, and adjust or stop treatment if your numbers or symptoms call for it. Talk with your healthcare provider to determine whether TRT is right for you.
Many men report improvements in energy and mood within a few weeks, with more complete changes in body composition and libido developing over a few months. Individual results vary based on health history, adherence to the treatment plan, and other factors. Nothing on this page should be taken as a guarantee of results.
Cost depends on the delivery method, your dosing, and how much lab work your plan requires, so we quote it after your consultation rather than posting a single number that would not apply to most people. We will give you a clear, itemized figure before you commit to anything. Call (208) 690-2040 to ask.
